Pamela's 2nd Merge Request

Updated title lines, comment mark positions, separation characters, and spacing. Created a line for “Instructions”, cleaned up what was already in some examples, and added in what was needed for the examples that didn’t have any. Text above “Instructions” lines in comments sections still need tweaking when we do purpose and descriptions.

Compare with previous version

Made some commits to wrap the "Swim Lanes" instructions in line with other lines. Please check Stack and Queue examples 4a and 4b - there may be a carry over error (mine?) from a previous merge that swapped the content of the models. The queue schema is in the stack model and the stack behavior is in the queue model.
